IMHO, the 91-99 4.0L is closer to a true 3 into 2 into 1 header.
Yes it will fit the two wheel drive cars! I don't know who started the rumour that it won't! It absolutely will! I have one in my American! 
  It's not a bolt on to the 232-258 engine, but close. If you can weld and fabricate you can put it on. You can get flanges from Headers By Ed. or rework the 4.0L flanges. 
   Mine is on a 4.0L in a 69 American. I had two manifolds, one was severely cracked at the rear, so I cut it off at the 2-1 junction and reworked it to have a 2.5" outlet then extended this 2.5" pipe down to just behind the bellhousing. At that point I installed a collector flange and ran 2.5" exhaust to the rear of the car. 
  Note; Tried a glasspack at the rear stock location, real loud! I couldn't handle it. So, I picked up a second glasspack and installed it under the passenger seat area. I figured it would act as one long glasspack, No dice, still real loud! Third try was a quality 2.5" Turbo muffler at the rear position with the front glasspack still in place. This combo is acceptable for me noise wise. Just barely! The six with all that large pipe and practially straight thru exhaust will be loud!
