[BaadAssGremlins] Wheel lug removal

Some of the wheel lugs (not lug nuts) on the rear of my 77 hornet 
require replacement.  My question is, how do you get the old ones 
out?  Do you have to have them pressed out, or will a really big 
hammer work?  If they have to be pressed out, I assume that the rear 
axle needs to be taken out?
