On the left side it has GWC then 0256 then below that it has 3181884 and then T112 (some of the numbers may be wrong, a couple were hard to read). then on the right side, just below the spark plugs it has, as well as I could read the numbers, 804129. I found a receipt in the glove box that indicates that this motor was rebuilt by Ram Engine some time in the past but I have found a date on the receipt.

There is some kind of adaptor screwed on where the oil filter would normally go and the a hose runs from that to a canister on the fender (there is a picture of that on that site) that says Frantz Oil Cleaner. There is another hose that runs from that canister to the valve cover. I have never seen a setup like this before.

The frame is bent toward the left side (both sides). It appears to be just in front of where the front axle is mounted. Any ideas to fix this?

Take it to a good frame shop. There may be more damage than you can see.
