Greg: Sounds like you've got a bad switch. You should only have power going to one side of the motor at a time. What's happening is that you're getting power to both sides at the same time--so essentially the motor doesn't know what to do since it's being told to go in both directions at once. Try swapping out the switch for another one and see if that helps. Since you're getting actual power to the motor, your wiring appears to be OK. And, since you tested the motor with the battery charger, you know the motor is good, so that pretty much leaves the switch as the culprit.
